Change URL | WordPress.org

As you may see in the link, this page url reads http://consenthub.info/beautiful-portfolios-for-exceptional-creatives/eu-youth/

I want to change the url to consenthub.info/eu-youth. How can I do this? When I click on the permalink, it shows “eu-youth”. Not sure where the middle text is coming from and how to change it.
